This is My Story (TIMS) bridges the gap between clinicians and hospitalized patients unable to communicate. Through the program, caregivers provide audio recordings of loved ones sharing the patients’ stories.
The idea for TIMS began during the COVID-19 pandemic, when patients’ family members were not allowed in treatment areas and clinicians had little information on patients who could not speak for themselves.
Caregivers were asked 4 questions:
- How does the patient want to be addressed?
- What brings the patient joy?
- What does the medical team really need to know about this person in order to give the best possible care?
- What sustains the patient in times of trouble?
TAG team members developed a mobile application to facilitate the audio recording process by guiding caregivers to answer these four key questions, delivering actionable insights to the treatment team and enabling holistic patient care.
